As we look ahead to the 2026-2027 school year, the Annual Health Packet season is just around the corner. All student health forms are managed exclusively through August Schools, our secure platform integrated with Veracross. Lakeside discontinued use of Magnus Health in September 2025.

On April 21, you will receive an email from Lakeside School (info@lakesideschool.site) when your student’s Annual Health Packet opens. Forms can be accessed through a link in that email or by logging into the Veracross Parent/Guardian Portal and selecting the August Schools tile.

Know your deadline

• June 1, 2026: Health forms due for all NEW STUDENTS
• Aug. 1, 2026: Health forms due for all RETURNING STUDENTS

Submitting forms by the deadline allows the health office to review documentation, follow up if needed, and ensure students are safely cleared before school begins.

If your child will need an updated physical exam, medication authorization form, or action plan for the 2026-2027 school year, please schedule an appointment with your healthcare provider before Aug. 1.

Before your appointment, print any required forms from August Schools and bring them with you for your provider to complete. If you do not have access to a printer, please contact the health office at info@lakesideschool.site, and we will provide a hard copy.

Summer appointments fill quickly. Students cannot be cleared to attend the first day of school or participate in activities without completed health documentation and required consents.
